Your responses to this survey are anonymous!
There is no way that your responses can be linked to you.
 
Here's how we make sure:
You do not enter your name or employee id in the survey.
Your responses come directly to NDNQI®.
NDNQI will not give your hospital access to your individual responses.
NDNQI will provide your hospital a report of all RN responses.
This report:
•   Does not include responses from individual RNs.
•   Includes the average response of all RNs on each unit to items or groups of items asking about satisfaction, quality of care, floating or overtime.
•   Includes unit averages only for units with 5 or more responses.
•   Includes the average response of all RNs in the hospital to items that could identify individuals, such as age, sex, or education, as well as job plans.

Participation in this survey is voluntary. If you choose not to participate, your job will not be affected.
